  • The tables listed below represent the entire database as found in the 1st edition published in 2007 entitled ‘Clinical Care Classification (CCC) Manual: A Guide to Nursing Documentation.’ They consist of four tables in Microsoft Word and should be downloaded to obtain the entire CCC database. They include (1) CCC of Care Components, (2) CCC of Nursing Diagnoses, (3) CCC of Nursing Diagnoses Outcomes, (4) CCC of Nursing Interventions & Actions.

CCC of Nursing Diagnosis

The 21 Care Components classify the two interrelated CCC terminologies and serve as the standardized framework for them:

CCC of Nursing Diagnosis

Version 2.0: The CCC of Nursing Diagnoses consists of 182 (59 two-digit major categories and 123 three digit subcategories) that depict nursing diagnoses and/or patient problems:

CCC of Nursing Outcomes

The three (3) Expected Outcomes: (To Improve, To Stabilize, or To Deteriorate) and/or Actual Outcome Qualifiers (Improved, Stabilized, or Deteriorated) are presented as separate files and need to be combined with the 182 CCC of Nursing Diagnoses to prepare the 546 Expected Outcomes and/or Actual Outcomes.

CCC of Nursing Interventions

The CCC of Nursing Interventions consists of 198 (72 two-digit major categories and 126 three-digit subcategories) that depict nursing interventions, procedures, treatments, activities, and/or services. The CCC of 198 Nursing Interventions is modified by using one of four Intervention Action qualifiers representing 792 unique Nursing Intervention Action concepts: (1) Assess or Monitor, (2) Care /Direct or Perform, (3) Teach or Instruct, (4) Manage or Refer:

Comma Separated Value Files (CSV)

Tables are also available as Comma Seperated Value Files (CSV). All files are less than 200k with arbitrary sort orders. For more information about how these tables relate please read the information above, visit the framework page, or view the model. If you have Microsoft Excel on your computer the CSV files will open in Excel and the commas will be hidden, essentially replaced by rows and columns in Excel. However, if you prefer to get the CSV files with the commas visible you will need to open the files with a text editor like notepad (found under start/programs/accessories on Windows machines). If the files open directly in your browser, consider downloading them first. On a Windows machine you can right click on the link and choose ‘Save link as’

Revisions v1.0 to 2.0

Listed below are the revisions and/or changes from Clinical Care Classification (CCC) System Version 1.0 to the CCC System 2.0. The revisions were recommended by the CCC Scientific Advisory Committee and announced in February 2004. A copy of the CCC System Version 1.0 is available on request.

Update Format is Arranged by Version 1.0 Classification as Follows:

  • ADD Code/Term = Single Entry Represents New Code and Term (ALL CAPS)
  • DELETE Code/Term = Single Entry Represents Retired Code/ Term (Lower Case)
  • ADD Term = Single Entry Represents Expansion of a Term with a New Word
