About

The CCC system facilitates the electronic documentation of nursing plans of care (POCs).  It is recognized by the ANA as a nursing terminology/classification critical for representing clinical nursing practice in computer-based clinical information systems.

The framework behind the CCC system makes it possible to chart the patients condition, nursing actions, and patient outcomes in an electronic health record (EHR). The system was originally developed by a team of researchers, led by Dr. Virginia Saba, from 1988-1991 at Georgetown University School of Nursing.  It is mapped to SNOMED CT, LOINC and ICNP and is widely used around the world.
