Mobile Care Plan

Welcome to the CCC System Mobile Care Plan designed to create structured Nursing Care Plans. This tool has been adapted from the Code Builder on the main website, but the design has been simplified to display better, and run faster, on mobile devices.

This tool helps demonstrate how the CCC system can be integrated into clinical systems and used to document Care Plans ‘on-the-go’ or at ‘bedside’ following the six steps of the ANA Nursing Process, helping you select: S1) Care Component (assessment), S2) Diagnosis (diagnosis), S3) Goal (outcome identification), S4) Intervention (planning) with S5) Action Type (implementation) and on discharge S6) Actual Outcome (evaluation). Note that no results are stored. This is for demonstration purposes only.
